We email it before the visit, or you fill it in at the desk ten minutes early.
3
The first exam
Forty-five minutes: a talk, a full check, X-rays if needed, and your first chart.
4
Your plan
The dentist explains the chart and gives you a written plan and its cost. You decide what to go ahead with.
Reading your chart
Teeth are numbered 1 to 32, starting at your upper right wisdom tooth and ending at your lower right one. The marks follow one rule: blue is done, rose is planned.
An empty tooth
Checked, and nothing to do.
A blue square
A filling you already have. Blue always means work done before, here or elsewhere.
A blue outline
A crown you already have.
A rose square
A filling we plan to do. Rose always means planned.
A dashed ring
Something small to look at again next time, such as early wear or a shallow stain.
A cross
No tooth: never grown, or taken out.
